Common ECG Findings: When Should You Take Them Seriously?


An ECG (Electrocardiogram) records the electrical activity of the heart. Some ECG changes are harmless, while others may indicate a medical emergency.

Seek Immediate Medical Attention If ECG Shows:

1. Heart Attack (Myocardial Infarction)

Serious ECG findings:

  • ST-segment elevation
  • New left bundle branch block
  • Significant ST depression with symptoms

Warning symptoms:

  • Chest pain or pressure
  • Pain spreading to arm, jaw, neck, or back
  • Sweating
  • Shortness of breath
  • Nausea

2. Dangerous Arrhythmias

Examples:

  • Ventricular tachycardia (VT)
  • Ventricular fibrillation (VF)
  • Complete heart block

Symptoms:

  • Fainting
  • Severe dizziness
  • Sudden collapse
  • Palpitations with breathlessness

3. Atrial Fibrillation with Rapid Heart Rate

Take seriously if associated with:

  • Chest pain
  • Breathlessness
  • Low blood pressure
  • Stroke symptoms

4. Severe Bradycardia (Very Slow Heart Rate)

Especially concerning if:

  • Heart rate below 40 bpm
  • Dizziness
  • Fainting
  • Confusion

5. Long QT Syndrome

May lead to life-threatening arrhythmias.
Seek urgent evaluation if:

  • Fainting episodes occur
  • Family history of sudden cardiac death exists

6. Signs of Pulmonary Embolism

ECG may show:

  • Sinus tachycardia
  • Right heart strain patterns

Associated symptoms:

  • Sudden breathlessness
  • Chest pain
  • Coughing blood

ECG Findings That Often Need Follow-Up (Not Usually Emergencies)

  • Premature atrial contractions (PACs)
  • Premature ventricular contractions (PVCs)
  • Mild sinus tachycardia
  • First-degree heart block
  • Nonspecific ST-T changes

These still require medical review if symptoms are present or if they persist.

Red Flag Symptoms Regardless of ECG Result

Seek urgent medical care if you have:

  • Chest pain lasting more than 15 minutes
  • Sudden shortness of breath
  • Fainting
  • Severe palpitations
  • Sudden weakness or speech difficulty
  • Unexplained sweating with chest discomfort
